WESTFIELD HIGH SCHOOL SPRING TRIP COMMITMENT FORM
This form is due with a $150.00 (non-refundable) deposit no later than October 16, 2007, in order to reserve your student’s participation in the trip to New York.
Please carefully read the following, download and complete the the form. Parents and students must agree to the conditions set forth in this commitment form in order for a student to participate in the trip.
- We understand that the $150.00 deposit is non-refundable should the student be unable to attend the trip.
- We understand that if the student is unable to attend the trip, the amount of refund (beyond the non-refundable $150) will be dependant upon how much money has already been permanently committed on his or her behalf. The closer the date of the trip, the less likely that any refunds will be able to be issued.
- We understand that the student’s participation in the trip is entirely optional and is not required for participation in chorus, nor is the student’s grade contingent in any way upon participation.
- We understand that this trip is for educational/curricular purposes, that it is not a vacation, and that students, chaperones, and any other trip participants are bound by Fairfax County Policy to adhere to the school approved itinerary and are not to deviate from it in any way.
- We understand that participation in this trip is a privilege, not a right. Students who consistently demonstrate through poor behavior in chorus class or at Westfield High School that they lack the self-control and maturity to be trusted on a trip to New York City will not be able to participate in the trip. In such an instance, all committed money will be refunded to the student. This decision will be made at the recommendation of Ms. Lardin with WHS administrative approval.
- We understand that students are bound by FCPS regulations at all
times while on the trip.
We understand that if a student is in serious violation of school policies while on the trip, he or she may be sent home at the discretion of the school administration. - We understand that a school administrator will be on this trip.
